Polk County MiddleGPA School
Grading Scale: Quality Points:
A - 90- 100 A = 4.0
B - 80 - 89 B = 3.0
C - 70 - 79 C = 2.0
D - 60 - 69 D = 1.0
F - 0 - 59 F = 0
Students must maintain a 2.0 or higher GPA and/or no failing grade in a
subject to participate in extracurricular activities and to be promoted.
Receiving high school credit in middle school
KMS offers Algebra I for high school credit. Students must receive at least an 80% for each semester in order to receive high school credit. High school classes are graded numerically each nine weeks and the two nine weeks grades are averaged together as a semester grade.
Attendance
Students returning from an absence MUST bring in a note signed by the parent or guardian. The note needs to include the dates of the absence and the reason why the absence occurred. This note MUST be in the office within five (5) days of the absence for the absence to be excused. Students are responsible for requesting and making up missed work. Appropriate reasons for excused absences are: personal illness, death in the family, or doctor's appointments. Students missing more than three (3) consecutive days are required to have a note from a doctor. Sick, Death in family, Dr Appt.
Dress Code
Students at all Polk County elementary and middle schools wear uniforms. The uniform top must be a
school shirt or a collared shirt (red, white, or navy blue). Sweatshirts / fleece jackets worn over the
school shirt must meet the red, white, or navy blue requirement also. The uniform bottom may be shorts,
jeans, or skirts, but they must be plain denim. No patches, quilting, leather striping, etc. is allowed. All
shirts must be tucked into the bottom portion of the uniform and a belt is required. School shirts, s
weatshirts, and jackets may be purchased in the KMS office. P.E. uniforms may also be purchased in
the office. Bookbags / backpacks are NOT allowed.

Immunizations
Florida law requires all 7th grade students to show proof of the following immunizations before they
may attend 7th grade classes: a second MMR, a tetanus-diphtheria booster, and the hepatitis B series
(3 doses). Proof of the above listed immunizations must be provided to the school on a blue
immunization card, provided by a physician or the health department.
Student Planners
Communication between school and home is an essential part of the learning process. Therefore, every
student at KMS is provided one free "Student Planner" to help improve school/parent communication as
well as promote student achievement. In order for a student planner to be used effectively the student
must take the responsibility of correctly copying assignments into the planner, completing quality
assignments, and making sure the assignments are turned in on time. The parent's responsibility lies in
reviewing and initialing the "Student Planner" daily.
